We have provided a list of various forms and handouts that we use on a daily basis at the clinic for your convenience. Many of these need to be filled out, completed and signed before your animals are admitted to the hospital for surgery. Having the clients read, understand and complete these papers will really help facilitate the admission process. All legal medical forms must be signed by the legal owner or guardian that is over 18 years of age. If a minor will be dropping of your pet, please ensure that the forms are completed and signed, and that they bring them with them.
Here is a list of all the surgery options you need to be aware of for all elective surgical procedures. At the time of admission the staff will go through these and other options for you to either accept or decline and sign for.
